Shipping Rates based on total $ amount purchased

Would like to base shipping rate on total $amount ie:
$0.01 - $29.99 rate $3.00
$30.00 - $39.99 rate $4.00
$40.00 - $49.99 rate $5.00
$over $50.00 rate $6.00

Anyone know how to do this?
