Adding Members Error

When adding members, I get the following error; "Duplicate entry '1' for key 1". I have made several attempts to find my error but am not successful. Can someone help?

Hi,

this error means that you try to add record with the the same key value.
Please publish your project on Demo Account and open a ticket at http://support.xlinesoft.com sending a URL to your pages along with instructions on reproducing this error.
